Common use of Issuance of 1099 Forms Clause in Contracts

Issuance of 1099 Forms. After this Agreement has been executed and funds have been transmitted to Davia’s counsel at the address set forth in Section 4.4., Meridian shall cause three separate 1099 forms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2 (if any); (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2 (if any), whose address and tax identification number shall be provided to Meridian prior to any payments being due under this Agreement; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.3 and 4.2 (if any).

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2 (if any); (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2 (if any), whose address and tax identification number shall be provided to Meridian Xxxxxxxx Home prior to any payments being due under this Agreement; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.3 and 4.2 (if any).

Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2 (if any); (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2 (if any), whose address and tax identification number shall be provided to Meridian Travelon prior to any payments being due under this Agreement; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.3 and 4.2 (if any).

Issuance of 1099 Forms. After this Agreement has been executed and funds have been transmitted to Davia’s counsel at the address set forth in Section 4.4., Meridian and Davia has provided to GBG all necessary W-9 forms, GBG shall cause three separate 1099 forms to be issued, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2 (if any); (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections Sections 4.1 and 4.2 (if any), whose address and tax identification number shall be provided to Meridian GBG and Homestead prior to any payments being due under this Agreement; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.3 and 4.2 (if any).

After this Agreement has been executed and the settlement funds have been transmitted to Davia’s counsel at the address set forth in Section 4.4.counsel, Meridian AHI shall cause issue three separate 1099 forms to be issuedforms, as follows: (a) The first 1099 shall be issued to the Office of Environmental Health Hazard Assessment, X.X. Xxx 0000, Xxxxxxxxxx, XX 00000 (EIN: 00-0000000) in the amount paid pursuant to Sections 4.1 and 4.2 (if any)4.2; (b) The second 1099 shall be issued to Davia in the amount paid pursuant to Sections 4.1 and 4.2 (if any)4.2, whose address and tax identification number shall be provided to Meridian prior to any payments being due under this Agreementfurnished upon request; and (c) The third 1099 shall be issued to the Xxxxxxx Law Firm (EIN: 55-08-58910) in the amount paid pursuant to Section 4.3 4.2 and 4.2 (if any)Section 4.3.
