Limits of Confidentiality. As a client of JCS it is important for you to understand that limits to confidentiality do exist. We may use or disclose records or other information about you without your consent or authorization in the following circumstances, either by policy, or because legally required: 1. For treatment: We may use your PHI within our practice to provide you with mental health treatment, including discussing or sharing your PHI with other JCS therapists and interns. Example: discussing your treatment with another JCS therapist in order to facilitate your care. 2. To obtain payment for treatment: We may disclose your PHI to bill and collect payment for the treatment and services we provided you. Example: we might send your PHI to your insurance company in order to get payment for the health care services provided.
