MATERIALS TO BE UTILIZED IN THE CONDUCT OF EVALUATIONS. To the extent that each is available at the time when an evaluation is conducted, the following materials shall be utilized in the conduct of the evaluation of each Unit Member: 1. The supervisory Evaluation Form completed in accordance with the provisions of this Appendix; 2. Any materials or statements submitted by the Unit Member in accordance with the provisions of this Appendix; and 3. Materials contained in the Unit Member's official personnel file.
