Medicaid Disproportionate Share Hospital (DSH. Reports. HMO must file preliminary and final Medicaid Disproportionate Share Hospital (DSH) reports, required by TDH to identify and reimburse hospitals that qualify for Medicaid DSH funds. The preliminary and final DSH reports must include the data elements and be submitted in the form and format specified by TDH.
