Statistical Reports. We may: (a) compile statistical reports from System Data, Smart Data and other information relating to the performance, operation and use of the Services; and (b) use System Data and Smart Data from the Services for security and operations management, creation of statistical analyses, and for research and development purposes. We may make these reports and information publicly available provided that they will not incorporate Content or personal or Confidential Information in a form that could serve to identify you or any User. We retain all IPR in this data, these reports and information.
Statistical Reports. Where a Party does not ensure an acceptable level of compliance with Article 147(11), it shall, upon request of the other Party, collect and provide to the other Party on an annual basis statistics on its procurements covered by this Title. Such reports shall contain the information established in Annex XIII, Appendix 4.

Statistical Reports. Each month, by the 20th day, statistical reports in the format requested by the Coordinator shall be submitted by all participating libraries to the Cooperative Coordinator for the previous month. Notwithstanding the foregoing, the Coordinator shall have the authority to adjust the reporting frequency to periods which are more efficient and productive to the administration of the Cooperative.
Statistical Reports. Upon written request from the Union, the Employer shall provide statistical reports to the Union concerning the bargaining unit such as demographic reports, new hires, salary costs or other reasonable requests, provided such information is readily available. The Employer shall respond to such requests on a timely basis.
Statistical Reports. 12.2.1 HMO must electronically file the following monthly reports: (1) encounter; (2) encounter detail; (3) institutional; (4) institutional detail; and (5) claims detail for cost-reimbursed services filed, if any, with HMO. Encounter data must include the data elements, follow the format, and use the transmission method specified by TDH in the Encounter Data Submission Manual. Encounters must be submitted by HMO to TDH no later than 45 days after the date of adjudication (finalization) of the claims.
12.2.2 Monthly reports must include current month encounter data and encounter data adjustments to the previous month's data.
12.2.3 Data quality standards will be developed jointly by HMO and TDH. Encounter data must meet or exceed data quality standards. Data that does not meet quality standards must be corrected and returned within the period specified by TDH. Original records must be made available to validate all encounter data.
12.2.4 HMO must require providers to submit claims and encounter data to HMO no later than 95 days after the date services are provided.
12.2.5 HMO must use the procedure codes, diagnosis codes and other codes contained in the most recent edition of the Texas Medicaid Provider Procedures Manual and as otherwise provided by TDH. Exceptions or additional codes must be submitted for approval before HMO uses the codes.
12.2.6 HMO must use its TDH-specified identification numbers on all encounter data submissions. Please refer to the TDH Encounter Data Submission Manual for further specifications.
12.2.7 HMO must validate all encounter data using the encounter data validation methodology prescribed by TDH prior to submission of encounter data to TDH. 1999 Renewal Contract Xxxxxx Service Area August 9, 1999 107
Statistical Reports. Valley Transit agrees to provide the County information sufficient to complete the Calumet County semi-annual reports for submission to the Wisconsin Department of Transportation as a requirement of the Section 85.21 transportation assistance program. The information submitted must pertain to the service identified in this agreement and must be provided in a timely manner. Valley Transit will also provide to the County all other reasonable ridership or financial information which the County requests.

Provide PARTICIPATING MUNICIPALITIES with quarterly statistical reports, available from Eaton County Central Dispatch (ECCD), related to: total number of runs, number of runs within each municipality, enroute and response time, number of mutual aid responses into EAEMS service area, and other reports which become available from ECCD or EAEMS that provide information related to the efficiency of ambulance coverage plans.
Statistical Reports. The Corrections Special School District maintains reports related to grades, attendance and Tests of Adult Basic Education. In addition, CSSD will maintain data concerning gender, race, national origin and disability, which is outlined in the Student Records of the Operational Manual, Appendix III, section V, pg. 11. This process will be done in coordination with NHDOC; Offender Records Department, which maintains all data related to an offender in relation to their incarceration and personal history. The Corrections Special School District will annually on July 1st,submit to the Bureau of Special Education, Department of Education, the numbers of offenders enrolled in its high school education and career and technical education programs broken out by gender, age, race, primary language, type of disability if any, and completion rates.
Statistical Reports. HMO CANNOT SUBMIT NEWBORN ENCOUNTERS TO TDH UNTIL THE STATE-ISSUED MEDICAID ID NUMBER IS RECEIVED FOR A NEWBORN. HMO MUST MATCH THE PROXY ID NUMBER ISSUED BY THE HMO WITH THE STATE-ISSUED MEDICAID ID NUMBER PRIOR TO SUBMISSION OF ENCOUNTERS TO TDH AND SUBMIT THE ENCOUNTER IN ACCORDANCE TO THE HMO ENCOUNTER DATA SUBMISSION MANUAL. THE ENCOUNTER MUST INCLUDE THE STATE-ISSUED MEDICAID ID NUMBER. EXCEPTIONS TO THE 45-DAY DEADLINE WILL BE GRANTED IN CASES IN WHICH THE MEDICAID ID NUMBER IS NOT AVAILABLE FOR A NEWBORN MEMBER.
