Common use of Medical Consent Forms Clause in Contracts

Medical Consent Forms. Organization shall obtain for each minor attendee a medical consent form signed by a parent or legal guardian authorizing Organization’s employees or staff to take ill or injured Attendees for medical treatment. Organization shall provide signed consent forms to University upon request, and University shall retain such consent forms in the Office of Conference & Event Services to the extent required by University policy and applicable law.
