Medical Information Form. The Resident or the Responsible Party has arranged with a physician for a medical examination conducted within three (3) months prior to admission and has submitted NBOC, Inc./AL’s Medical Information Form completed by the physician. All information provided in the form is true, complete and current as of the date of this Agreement and will be updated prior to admission if requested by NBOC, Inc./AL staff or if there are changes in health status before admission.
