Common use of Notice – Access by Individual Clause in Contracts

Notice – Access by Individual. VENDOR agrees to notify Community in writing within three (3) business days of any request by an individual for access to the individual’s PHI or EPHI and, upon receipt of such request, direct the individual to contact Community to obtain access to the individual’s PHI. Upon request by Community and within three (3) business days of the request, VENDOR agrees to make available PHI and EPHI to Community in accordance with 45 C.F.R. § 164.524.
