Notification to Enrolled Employees Requirements. In addition to its other obligations hereunder to provide notifications to Members, Employer Group shall be responsible for informing and shall inform Enrolled Employees of the following: (1) conditions of eligibility for enrollment in Plan; (2) when coverage under this Agreement becomes effective and terminates; (3) any continuation coverage under the Consolidated Omnibus Budget Reconciliation Act of 1985, as amended ("COBRA"); (4) any continuation coverage under Plan purchased or arranged for by Employer Group; and (5) any change in the terms or conditions of this Agreement.
