Number of Shares to be delivered to Investor at or about the Closing: 14,646 16,556 Purchaser Price per Share: $ 17.07 12.08 Aggregate Purchase Price: $ 250,007.22 199,996.48 This Adoption Agreement (“Adoption”) is executed pursuant to the terms of the Stockholder Agreement Agreement, dated as of September 30April 17, 20102012, a copy of which is attached hereto (as amended, restated, supplemented or otherwise modified or supplemented from time to time, the “Stockholder Agreement”), by the undersigned (the “Undersigned”) executing this Adoption. By the execution of this Adoption, the Undersigned agrees as follows:

By the execution of this Adoption, the Undersigned agrees as follows:

By the execution of this Adoption, the Undersigned agrees as follows:
