OSU agrees to provide: 1. An annual stipend commensurate with institutional guidelines and post graduate year that is uniform among clinical departments, pursuant to recommendations made by the Graduate Medical Education Committee and approved by the Associate Xxxx for Graduate Medical Education and the Chief Clinical Officer. Outside professional activity performed for hire (moonlighting) is not covered with the exception of services provided at OSU-UH locations and previously approved in writing by the Chief Medical Officer. 3. Group Term Life insurance in an amount of two and one half times annual stipend, to a maximum of $250,000. Optional Voluntary Group Term Life Insurance (VGTLI) coverage for yourself, your eligible spouse and/or dependent children up to age 26. You pay the full cost of VGTLI coverage. 4. Eligibility to enroll in the OSU-sponsored health insurance plans, which includes medical, dental and vision options. Premiums and coverage vary by plan and coverage level. 5. Upon employment, Resident will begin accruing sick leave benefit hours. This benefit provides the Resident full pay for up to the total number of hours that have been accrued. Xxxx leave does not accrue during unpaid leaves of absences. There is no limit on the number of hours that may be accrued. Full-time employees (100%) accrue 120 hours of sick leave per year. Appointments less than 100% accrue sick leave at a pro- rated amount according to time actually worked. No moonlighting while on sick leave is permitted. 0. The FMLA provides protections for employees to ensure that when the leave is concluded, the university provides job protected individual will return to the same or an equivalent position with equivalent benefits, pay and other terms and conditions of employment. a. Per OSU policies, a Resident must have at least twelve (12) months of service with OSU and must have worked at least 1,250 hours during the twelve (12) month period immediately preceding the commencement of leave to be eligible for Family and Medical Leave (FML) benefits. Residents with less than twelve (12) months of service are not eligible for FML under OSU policies and federal law, but may have medical or other leave arranged on an as needed basis. b. Although health plan benefits provided to eligible employees for up to 12 workweeks (480 hours) of leave an employee will be maintained during a 12 month period based of FML, benefits are subject to changes that occur within the group plan while the individual is on leave. c. FML may be taken for the following qualifying eventsevents as defined in OSU Human Resources Policy 6.05: ▪ A serious personal health condition of the employee that prevents an employee from performing his or her job as certified by a health care provider; ▪ Birth of a child; ▪ Placement with the employee of a child for adoption or xxxxxx care; ▪ Care of a child during the first year following birth, adoption or xxxxxx care placement; ▪ Care for an immediate family member who has a serious health condition as certified by a health care provider.
