All services provided to Medi-Cal beneficiaries must be preauthorized by the Mendocino County Behavioral Health and Recovery Services Managed Care Plan per the Mendocino County Mental Health Plan Organizational Provider Manual and per the Procedure for Obtaining Therapeutic Behavioral Services (TBS) Authorization. 2. CONTRACTOR will administer Child and Adolescent Level of Care Utilization System (CALOCUS) at the time of the first assessment. The level of services requested should be consistent with the CALOCUS score. 3. CONTRACTOR will ensure that the Brief Strengths and Difficulties Questionnaire (Attachment 1) is administered by the fourth (4th) session after admission, again after ninety (90) days, and then biannually throughout the course of treatment. The findings will be submitted to the COUNTY upon request. 4. CONTRACTOR will ensure that the attached Beneficiary Satisfaction Survey (Attachment 2) is administered at ninety (90) days from the date of admission and then annually. 5. CONTRACTOR shall provide Mental Health Services (MHS) to a target population consisting of children and youth up to age 21 who are on full-scope Medi-Cal and meet the criteria for specialty mental health services. This will include children ages 0-5 and transition age youth 18-21. 6. CONTRACTOR will provide services at its Ukiah office site, at schools, and in the community, including, when indicated, in client’s homes. 7. CONTRACTOR shall provide services up to a projected number of 165 clients 8. Whenever possible and clinically appropriate, CONTRACTOR shall provide treatment incorporating evidence-based practices, including Parent Child Interactive Therapy (PCIT), Trauma-focused Cognitive Behavioral Treatment, Motivational Interviewing and Positive Parenting Program (Triple P). 9. CONTRACTOR shall provide the following services to Medi-Cal beneficiaries, and to non-Medi-Cal beneficiaries as funding allows: A. An intensive outpatient dual diagnosis adolescent treatment program for youth enrolled in New Beginnings. B. Specialized outpatient mental health treatment program for children and youth under age 18 who are victims/ witnesses of crimes. C. Specialized outpatient mental health treatment program for children and youth under age 18 who are victims of child sexual abuse. DEFINITION OF SERVICES D. Specialized outpatient mental health services for homeless youth, age 16-21, in connection with CONTRACTOR’s residential transitional living program. E. Standard specialty outpatient mental health therapy services. 10. All services provided to Medi-Cal beneficiaries must be preauthorized by the Mendocino County Behavioral Mental Health and Recovery Services Managed Care Plan per the Mendocino County Mental Health Plan Organizational Provider Manual and per the Procedure for Obtaining Therapeutic Behavioral Services (TBS) AuthorizationManual. 2. Expected Scope of Services for Transitional-Age Youth (TAY) and adults is 4-6 clients and targets the ages of 21-24 who will mostly be self referrals, but may come from law enforcement and other agencies for persons who have a history of prior mental health needs. (TAY population up to age 21 is served under contract # BOS 11-100). 3. CONTRACTOR will administer Child and Adolescent Level of Care Utilization System (CALOCUSLOCUS) at the time bi- annually or more frequently if requested by County to all individuals who are submitted for authorization of the first assessmentservices. The level of services requested should be consistent with the CALOCUS LOCUS score. 3. CONTRACTOR will ensure that the Brief Strengths If there is an exceptional circumstance, where a Clinician determines a higher need for services, a new LOCUS score and Difficulties Questionnaire (Attachment 1) is administered by the fourth (4th) session after admission, again after ninety (90) days, and then biannually throughout the course of treatment. An intensive outpatient dual diagnosis adolescent treatment program for youth enrolled in New Beginningsspecified under the Uniform Method of Determining Ability to Pay (UMDAP) as developed by the State Department of Mental Health, as updated by DMH or successor agency. B. Specialized outpatient TAY WELLNESS PROGRAM SERVICES The CONTRACTOR, in collaboration with Health and Human Services Agency (HHSA) Behavioral Health and Recovery Services, shall provide a supported, independent living experience, through the RCS TAY Wellness Program, for up to six to eight eligible Mendocino County Transition Age Youth (TAY), age 18-24 who are specifically in need of mental health treatment program for children services. The TAY Wellness Program is a ‘Full Service Partnership’ providing an array of services. Participants will live independently in apartments, houses, or room rentals with the support and youth under age 18 who are victims/ witnesses guidance of crimes. C. Specialized outpatient mental health treatment program for children and youth under age 18 who are victims of child sexual abuse. DEFINITION OF SERVICES D. Specialized outpatient mental health services for homeless youth, age 16-21, in connection with the CONTRACTOR’s residential transitional living program. E. Standard specialty outpatient mental health therapy services. 10. CONTRACTOR will provide the following specific services and activities as identified on the client’s treatment plan and authorized by COUNTY: A. Assessment Evaluation B. Plan Development and Case Conference Services C. Individual, Family’S supervising/support staff from its Transitions Program, and Group Therapy D. Collateral mentors within the community to enable them to acquire and Collateral Family Therapy practice skills which will prepare them to live successfully on their own. Services E. Rehabilitation provided to TAY will be client-driven, strength-based, recovery-oriented and Rehabilitative Group Servicesculturally-competent. The goal of the program is to ensure that these young adults stabilize their housing situations, increase their employability, complete their GEDs and/or enroll in an institution of higher learning, and access community resources that support their physical health and their social and emotional well being. The CONTRACTOR shall make emotional support and crisis intervention available on a twenty-four-hour-a-day, seven- days-per week basis through its Transitions Program and its Children’s Therapeutic Services (CTS).

The results of the surveys should be discussed with the Clinical Supervisor. If a client indicates through the Survey that services are not being provided in a competent and ethical manner, the Beneficiary Protection: Grievance/ Appeal/ Problem Resolution policy should be followed to ensure that the matter is addressed. (Exhibit E-4). The surveys should be retained by the CONTRACTOR and submitted to the COUNTY upon request. 5. CONTRACTOR will ensure that the “Strengths and Difficulties Questionnaire” (Exhibit E-5) is administered at the commencement of treatment, ninety (90) days from the date of admission and then annually. biannually to coincide with the TAR submissions. The scores will be entered on the Treatment Authorization Request (TAR) (Exhibit E-6). If there are discrepancies between the Strengths and Difficulties scores and the treatment request, the findings will be discussed with the COUNTY and appropriate changes will be made to the client plan (Exhibit E-7). Services will be terminated if the client no longer meets medical necessity. Surveys will be retained by the CONTRACTOR and will be available to the COUNTY within five (5) business days of the request. Whenever possible and clinically appropriate, CONTRACTOR shall provide treatment incorporating evidence-based practices, including Parent Child Interactive Therapy (PCIT), Trauma-focused Cognitive Behavioral Treatment, Motivational Interviewing and Positive Parenting Program (Triple P). 9. CONTRACTOR shall provide the following services: a. Assessment /Evaluation includes a clinical analysis of the history and current status of a client’s mental emotional or behavioral disorder, relevant cultural issues and history, diagnosis and use of testing procedures. The process also includes an appraisal of the individual's community functioning in several areas including living situation, daily activities, social support systems and health status. The results of the assessment process are documented in a formal assessment tool and are used to develop a Client Plan. b. Client Plan Development includes the development of client plans, approval of client plans, and/or monitoring of the child’s progress. c. Linkage/Case Management include the identification and pursuit of resources including but not limited to interagency and intra-agency consultation, communication, coordination and referral, monitoring service delivery to ensure the child’s access to services to Medi-Cal beneficiariesand the service delivery system, and to non-Medi-Cal beneficiaries as funding allows: A. An intensive outpatient dual diagnosis adolescent treatment program for youth enrolled in New Beginnings. B. Specialized outpatient mental health treatment program for children monitoring of the child’s progress and youth under age 18 who are victims/ witnesses of crimes. C. Specialized outpatient mental health treatment program for children and youth under age 18 who are victims of child sexual abuseplan development. DEFINITION OF SERVICES D. Specialized outpatient mental health d. Case Conferences is a service activity with multiple service providers which consists of development of client plans, approval of client plans and/or monitoring of a beneficiary’s progress. e. Individual Therapy is designed to remedy, ameliorate or stabilize a diagnosed condition or assist a child in progressing developmentally as individually appropriate. Individual therapy provides focused attention with emphasis on cognitive behavioral theory, helping the child to understand how thinking and behavior impacts emotions. This service focuses primarily on symptom reduction as a means to improve functional impairments. f. Family Therapy is focused on symptom reduction in the child as a means to improve functional impairments. If it is difficult for family members to travel to the Tapestry treatment site, transportation is provided or the family therapy may be provided in the home. Family Therapy services provided by Tapestry include (but are not limited to) two specific evidence-based practices, as follows: 1) Parent Child Interaction Therapy (PCIT) is an empirically-supported treatment with emphasis on improving the quality of the parent-child relationship and changing parent-child interaction patterns. Parents are taught specific skills to establish a nurturing and secure relationship with their child while increasing their child’s pro-social behavior and decreasing negative behavior. Parent Child Interaction Therapy (PCIT) is available to families with children ages two through eight. This service is an integral component of Mendocino County’s Early Mental Health Project. The service is available in English and Spanish and is offered in Ukiah at Tapestry’s PCIT facilities, through Family Connections at the Willits Family Resource Center, at the Laytonville Family Resource Center and in Covelo. 2) Triple P (Level 5) is a family support strategy that aims to prevent severe behavioral, emotional and developmental problems in children by enhancing the knowledge, skills and confidence of parents. Triple P parenting services are available for homeless youthfamilies with children ages 2 through 16; services are provided in a group setting or with individual families. 3) All family therapy, age 16including PCIT and Triple P, are offered in the Tapestry clinics or in the home. DEFINITION OF SERVICES g. Group Therapy is focused on symptom reduction as a means to improve functional impairments. Group Therapy, usually occurs on a weekly basis, is utilized for building self-21esteem, learning social skills, and implementing behavior modification strategies. Group therapy also helps each child connect with and express feelings encountered in connection with CONTRACTOR’s residential transitional living the other parts of his/her treatment program. E. Standard specialty outpatient mental health therapy services. 10. CONTRACTOR will provide the following specific services and activities as identified on the client’s treatment plan and authorized by COUNTY: A. Assessment Evaluation B.
