Other Terms & Conditions. (a) This Appendix establishes a process for direct access to a family physician at QFHT. Employees must make an appointment at a time and location determined to be suitable by QFHT (i.e., this is not a walk-in clinic service). (b) Employees and their family members who make and attend appointments at QFHT should have their health insurance information readily available. Employees and their family members are responsible for ensuring family medical services are paid for, whether under the Ontario Health Insurance Plan or otherwise. (c) The parties to the Agreement agree that this is a Pilot Project and that it may be cancelled by the Employer. If the Pilot Project is cancelled, no Employee enrolled in the Pilot Project shall lose access to the QFHT.
