Outcome Report Clause Samples

An Outcome Report clause requires one party to provide a formal summary or documentation of the results achieved under the agreement. Typically, this involves detailing the completion status of deliverables, performance metrics, or the impact of services rendered, and may include timelines for submission and required content. The core function of this clause is to ensure transparency and accountability by formally recording outcomes, thereby enabling both parties to assess whether contractual obligations have been met.
Outcome Report. Upon completion of Job Retention Services, the Contractor shall submit a written narrative report on the SDOR form indicating: A. Dates for each of the job site Job Retention services provided; B. How Contractor assisted the Customer in learning job tasks, appropriate work behaviors and meeting the Employer’s expected level of job performance; C. If any services were provided remotely, describe how services were provided and how they assisted the Customer in retaining their job; D. The Customer has retained continuous employment for at least ninety (90) calendar days after Job Retention services are authorized; E. Describe employer’s satisfaction with the Customer’s performance and if any additional support is required; and F. Any additional details requested on the SDOP.
Outcome Report. ‌ (a) The Applicant must prepare an outcome report containing reasonable details of the operating activities of the Applicant relating to the Project, including a financial acquittal for all expended funds.(Outcome Report). (b) The Outcome Report must include: (i) an executive summary (maximum 4 pages); (ii) an introduction/background detailing the rationale, aims and objectives of the Project; (iii) A brief description of the research methods; (iv) the outcomes, achievements and implications of the Project; (v) a copy of any products and resources produced as part of the Project; (vi) a copy of any media releases relating to the Project; (vii) details of any media coverage of the Project; (viii) any problems or risks encountered by the Applicant in conducting the Project; (ix) details of the involvement of other agencies in the Project and the extent of such involvement; (x) a plan for further developing the Project after the End Date; and (xi) a financial acquittal for all expended funds.
Outcome Report. Upon completion of Intensive Training Services, the Contractor shall submit a written narrative report on the SDOR form indicating: A. Dates for each of the Intensive Training Services provided; B. Summary of services provided to Customer and what skills Contractor assisted the Customer in learning; C. Describe how Customer achieved stability: How Contractor was able to reduce job coaching to the minimum level needed by the Customer, and what supports they helped set up in the workplace, if any. D. If any services were provided remotely, describe how services were provided and how they assisted the Customer in retaining their job. E. Name of the entity providing the long-term employment supports. F. Any additional details requested on the SDOP.
Outcome Report. Upon Completion of the WBLE, the Contractor shall submit a written report on the Service Delivery Outcome Report (SDOR) form indicating the following for each WBLE site(s): Dates and hours of all activities focused on securing and monitoring WBL; Specific area of interest located; Name of the Contractor's staff person providing these services; Overall summary of the WBLE and verification that CRP has reviewed the summary with the student; and Specific recommendations for further student exploration and experience.
