Common use of OŚWIADCZENIA Clause in Contracts

OŚWIADCZENIA. 1. Parent(s) /Guardian(s) declare that they are the statutory representatives of the Student(s) being under their parental authority/custody and that their parental authority / custody is not restricted or suspended in any way. In the event of any change in this regard, Parents /Guardian(s) shall inform the School immediately. 2. The Parent(s) declare that they are aware that both parents have both the right and the duty to exercise parental authority unless it has been restricted, suspended or revoked by law. This means, in particular, that each parent, even if not a party to the Agreement, has the right and the duty to obtain information about and take an active part in the school activity of the Student(s). 3. I, the undersigned, acting as the Student(s) Parent/Guardian, hereby declare that I am acting with the consent of the other parent/guardian. I declare that the other parent/guardian has consented to the enrollment of the student in the ISK. At the same time, I release the ISK from any liability towards the other parent/guardian that may arise in connection with the enrolment of the Student(s) in the School without the consent of both parents / guardians. 1. Rodzic(e)/Opiekun(owie) oświadcza(ją), że jest/są przedstawicielami ustawowymi Ucznia/ Uczniów, pozostającego/ych pod jego/jej/ich władzą rodzicielską /opieką, a jego/jej/ich władza rodzicielska /prawo do opieki nie jest w żaden sposób ograniczona/e lub zawieszona/e. W przypadku jakichkolwiek zmian w powyższym zakresie Rodzic(e)/Opiekun(owie) zobowiązani są niezwłocznie poinformować́ o tym Szkołę. 2. Rodzic(e) oświadcza(ją), iż ma(ją) świadomość, iż oboje rodzice mają zarówno prawo jak i obowiązek wykonywania władzy rodzicielskiej, o ile nie została ona prawnie ograniczona, zawieszona lub odebrana. Co w szczególności oznacza, iż każdy rodzic, nawet nie będący stroną Umowy ma prawo a zarazem obowiązek do uzyskiwania informacji o aktywności szkolnej Ucznia / Uczniów i do brania w niej czynnego udziału. 3. Ja, niżej podpisany / podpisana, działając jako Rodzic / Opiekun Ucznia/ Uczniów niniejszym oświadczam, iż działam za zgodą drugiego rodzica / opiekuna. Oświadczam, iż drugi rodzic / opiekun wyraził / wyraził/a zgodę na zapisanie Ucznia/ Uczniów do ISK. Jednocześnie zwalniam ISK z wszelkiej odpowiedzialności względem drugiego rodzica / opiekuna, która mogłaby powstać w związku z przyjęciem Ucznia/ Uczniów do Szkoły bez zgody obojga rodziców / opiekunów.
