Participant Satisfaction Survey. a) Annually distribute the appropriate Participant Satisfaction Survey to all congregate and home-delivered nutrition services participants to obtain the views of participants about the services received. b) The surveys will be supplied by the County. c) Subrecipients have the flexibility to add questions or include a needs assessment. Additions to the survey must be approved by the County prior to dissemination. d) Survey finding must be tabulated using the Participant Satisfaction Survey Results Workbook and submitted to the County via email to the XxXXxxxXxxx@xxxx.xxxxx.xxx on or before May 30th along with a summary detailing a plan of action addressing relevant concerns of participants to improve program services.
