Plan Coverage. The City will provide a medical/dental, vision, and prescription drug insurance plan for all eligible employees including all bargaining unit members and their eligible dependents.
Plan Coverage. Life insurance is provided through a $20,000 term policy on the employee only with the option to purchase family ($10,000 term policy on an employee's spouse and $5,000 term policy on the employee's dependents). Employees also have the option of purchasing a supplemental term policy solely at the expense of the employee.
Plan Coverage. The Employer shall provide a plan for coverage as outlined in 35.02, 35.03 and 35.04. Membership shall be optional for regular employees, who have completed probation and who are normally scheduled to work fifteen (15) or more hours per week. Eligible employees must opt to enrol for coverage on or before the next January 1st. Thereafter, they may change their option for coverage once per year, on January 1st. For other employees, coverage shall commence following the completion of probation, if before the 15th of the month; if after the 15th of the month, at the start of the next month. Employees who opt to receive dental plan coverage must also take extended health benefit coverage and vice versa.
Plan Coverage. EPET and the Board of Education agree to use EBC for insurance administration. Medical benefits and coverage for members shall remain at existing plan levels as previously provided by Blue Cross/Blue Shield during the 2019-2020 school year. Dental benefits and coverage will also be comparable to the coverage previously provided during the 2019-2020 school year.
Plan Coverage. The Board shall provide hospital, drug, medical insurance, dental and vision plan coverage. The Board shall pay one hundred percent (100%) of the premium costs for all members including the member’s spouse and dependant children in accordance with the following policies:
a) Manulife Financial, or equivalent, semi-private
b) Manulife Financial, or equivalent, extended health care plan 10/20 and thirty-five ($0.35) cent prescription plan. A dispensing fee cap of ten ($10.00) dollars shall be introduced for all prescription drugs. This plan shall include Manulife Deluxe Pay Med benefit or equivalent and the following:
i) Chiropractic Services: $75.00 per visit, to a maximum of $1,000.00 annually to be used in conjunction with the member’s annual OHIP eligibility.
ii) Physiotherapy: maximum $75.00 per visit, to a maximum of $1,000.00 annually.
iii) Massage Therapy: maximum $75.00 per visit, to a maximum of $1,000.00 annually.
c) The Board will continue to remit the required payroll tax known as the Employer Health Tax.
d) Manulife Financial, or equivalent Dental Plan 15, at the current O.D.A. rates. Maximums of $3,000.00 for specialized services.
e) Manulife Financial, or equivalent, Vision Care, $385.00 every twenty-four (24) months. This benefit may also be used for laser eye correction procedures and eye examinations.
f) The Board will pay one hundred (100%) percent of the benefit premium costs for a), b) d) and e) for the spouse and dependent children of a member who has died prior to retirement, for a period of twenty-four (24) months from the date of the member’s death. Such coverage will terminate if the spouse turns sixty-five (65) years of age, remarries or is eligible for coverage through another employer.
x) Xxxxxx/Widowers and dependents of Police Officers killed in action while in the discharge of their duties or from injuries received in the service of the Board as a Police Officer, and in either case for which the compensation award is made by the Workplace Safety and Insurance Board, the Board will have the above named included in the benefit plans above (a, b, d and e). Coverage is for owned or rented residential property, not commercial property or premises converted into a business. You acknowledge receipt of a copy of this Agreement which outlines your equipment coverage under the Plan.

Plan Coverage. From and after August 1, 2006 and until the earlier of March 31, 2013 or such time as another employer makes available to Guild medical and dental coverage comparable to that which the Company currently provides to Guild, the Company, under COBRA, but at its expense, shall provide medical and dental coverage for Guild under, and subject to the terms and conditions of, such group insurance plans as the Company now and in the future makes available generally for its employees. Nothing in this Section 3 shall be construed to require the Company to institute or maintain any or any particular benefit plan, program or policy. If and to the extent that this Section 3 conflicts with any COBRA notice or other document issued by the Company at any time, the provisions of this Section 3 shall prevail.
Plan Coverage. During the Term, we will attempt to contact you to arrange for an Authorized Repair Technician to provide one or more of the services, as applicable, set forth in this section subject in all cases to the terms of this Contract, including this section and the limitations of liability set forth in SECTION 9. If you are not the owner of the Covered Property, you represent to us that you are authorized to enter into this Contract. The coverage of said plans is providing benefits in the following matter: (1) plumbing plan per service address, (1) electrical plan per service address, and (1) HVAC plan per HVAC System per address as defined by the service provider. The service provider maintains the right to modify plans per system as it sees fit to adequately cover the home.
