Primary Care Provider. Pharmacy must notify the patient’s primary care provider, as provided by the individual or the individual’s agent, within fourteen (14) days of the administration of the vaccination. This notification must include the name and address of individual, age if under fourteen (14) years old, name of primary care physician if provided, date vaccine given, amount administered, manufacturer, lot number, expiration date, site of vaccination and route of administration, and the name, address and title of the person administering vaccine.13
