Common use of Primary Care Clause in Contracts

Primary Care. 2.5.2.1. The PCP must: 2.5.2.1.1. Provide primary medical services, including acute and preventive care; 2.5.2.1.2. Refer the Enrollee, in coordination with the ICT and in accordance with the Contractor’s policies, to Covered Service providers, as medically appropriate; and 2.5.2.1.3. Lead the ICT, together with the Care Coordinator, and if indicated, with the behavioral health clinician

Primary Care. 2.5.2.12.6.1.1. The PCP must: 2.5.2.1.12.6.1.1.1. Provide primary medical services, including acute and preventive care; 2.5.2.1.22.6.1.1.2. Manage, in coordination with the ICT, an Enrollee’s medical situations; 2.6.1.1.3. Oversee, in coordination with the ICT, an Enrollee’s use of specialists and inpatient care; and, 2.6.1.1.4. Refer the Enrollee, in coordination with the ICT and in accordance with the Contractor’s policies, to Covered Service network providers, as medically appropriate; and 2.5.2.1.3. Lead the ICT, together with the Care Coordinator, and if indicated, with the behavioral health clinician.
