Privacy & Security Incidents; Breaches. 1. Contractor shall immediately report to the Covered California Privacy Officer at: XxxxxxxXxxxxxx@xxxxxxx.xx.xxx any actual or suspected Privacy or Security Incidents involving PII created or received under this Agreement. Contractor’s report (“Initial Report”) shall be provided on that certain Privacy & Incident Intake Form, developed by the Covered California Privacy Office and attached and incorporated by reference herein as Attachment 2. Contractor’s Initial Report shall, to the maximum extent possible, include the following information to the extent it is known at that time:
