Procedures for Completion of Evaluation Forms. 5.3.1 Evaluations shall be made in quadruplicate on employer forms. The original shall be sent to the Personnel File; one copy shall be kept in the files of the site manager; one copy in the files of the immediate supervisor if other than the site manager; and one copy shall be given to the employee. Employees are encouraged but not required to complete goals and objectives.
