PROJECT DATABASE. A. Grantee shall maintain a secure project database for all project activities including, but not limited to, the Clients served, services delivered directly to Clients, Client demographic data, and Client intake and assessment data. B. The project database must include, at a minimum, the elements required to be reported in C. Each Client must be assigned a unique identifier. The Client’s social security number protected health information, or other personally identifiable information must not be used as an identifier.
