Proof of Completion. All CDBG expenditures shall be included in the Grantee’s single audit for each fiscal year in which $750,000 or more in federal funds are expended. Grantees are required to have an audit performed in accordance with Code of Federal Regulations Title 2, Subtitle A, Chapter II, Part 200, Subpart F (Uniform Administrative Requirements, Cost Principles and Audit Requirements for Federal Awards—Audit Requirements), which begins at 2 CFR, Section 200.500. Grantees must submit the audit to the Division within 30 days of the date the audit is approved by the New Mexico State Auditor. The threshold of $750,000 is effective in fiscal years beginning on or after December 26, 2014. CDBG must be listed in the Schedule of Expenditures of Federal Awards (SEFA) as a funding source. The Division retains the right to recover funds from the Grantee for any disallowed costs based on the results of any interim or the final audit.

Proof of Completion. The Recipient shall provide the Province with proof that that the Project is complete and may be used for its Intended Use.
Proof of Completion. Reimbursement shall be provided only after the teacher has provided the Superintendent with proof that he/she has earned a grade of “C” or better, or “pass” in a “pass/fail” option. For conferences, workshops and seminars, the teacher shall provide proof of satisfactory completion of the Course (e.g., proof of attendance). Grade reports/proof of completion shall be filed with the Superintendent within three months of the completion of the Course. Teachers who fail to meet the proof of completion requirements outlined herein shall be required to reimburse the District for any payment made in connection with the Course, or shall be required to pay for the costs associated with the Course themselves if no District payment has been made. If reimbursement is required, payment must be made in full (not through payroll deduction) within thirty (30) days of being notified of the reimbursement amount due unless an alternative payment arrangement is approved by the Superintendent. At his/her sole discretion, the Superintendent may waive the reimbursement requirement in extenuating circumstances.
Proof of Completion. The Driver Education Specialist must provide a test receipt, examination to each Applicant who passes each required examination as proof of completion. a. The receipt must be in a format designated by the Department and must comply with procedures to confirm that an Applicant presenting the receipt to the Department is the same person to whom the test receipt was issued by the Driver Education Specialist. b. Only the Driver Education Specialist who personally administered the Department knowledge and/or skills exams may provide an Applicant with a test receipt. c. No such receipt may be given by or on behalf of other instructors nor may a Driver Education Specialist authorize another person to do so on their behalf.

There are instances where proof of completion is not available; therefore a screenshot of progress benchmarks or a written letter from an instructor will suffice.
Proof of Completion. Upon completion of the course, a copy of the grade sheet or certificate with a grade of C or better, together with original receipts and proof of payment by the employee unit member of tuition, fees, etc., shall be submitted to Human Resources for processing. Any and all reimbursement must be returned to the City in full if the employee unit member does not achieve at least a C grade. A copy of the tuition information is placed in the employee's unit member’s personnel file.
