Common use of Protected Health Information Clause in Contracts

Protected Health Information. “Protected Health Information” or “PHI” has the same meaning as the term “protected health information” in 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to information received by Business Associate from, or created or received by Business Associate on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “(PHI” has ) shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof HIPAA Customer.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “"protected health information” " in 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained, or transmitted by Business Associate from, from or created or received by Business Associate on behalf of, of Covered Entity, when providing the Services Entity pursuant to the Underlying Agreementthis Addendum.

Protected Health Information. “Protected Health Information” or “(PHI” has ) shall generally have the same meaning as the term “protected health information” in at 45 C.F.R. CFR § 160.103, provided thatand shall include any individually identifiable information that is created, for the purposes of this Addendumreceived, “PHI” is limited to information received by Business Associate frommaintained, or created or received transmitted by Business Associate on behalf ofof Covered Entity that relates to an individual’s past, Covered Entitypresent, when providing the Services pursuant to the Underlying Agreementor future physical or mental health, health care, or payment for health care, whether such information is in oral, hard copy, electronic, or any other form or medium.

CFR § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR §160.103, provided thatVersion: March 10, for the purposes of this Addendum, “PHI” is 2022 limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant including but not limited to the Underlying AgreementElectronic Protected Health Information.

Protected Health Information. “Protected Health Information” (or “PHI” has ”) shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” 160.103 and is limited to information received by Business Associate the Protected Health Information that (Vendor) receives from, or created or received by Business Associate creates, receives, maintains and/or transmits on behalf of, Customer through the performance of the Covered Entity, when providing the Services pursuant to the Underlying AgreementServices.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant including but not limited to the Underlying AgreementElectronic Protected Health Information.

Protected Health Information. “Protected Health Information” or “PHI” has PHI shall have the same meaning as the term “protected health information” in 45 C.F.R. § §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained, or transmitted by Business Associate from, from or created or received by Business Associate on behalf of, Covered Entity, when providing the Services of covered entity pursuant to the Underlying this Agreement.

C.F.R § 160.103164.501, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, Covered Entity, when providing of the Services pursuant to the Underlying AgreementHealthcare Provider. PHI includes electronic Protected Health Information.

Protected Health Information. “Protected Health Information” or “PHI” has the same meaning as the term “protected health information” in 45 C.F.R. § 160.103, provided that, for the purposes of this AddendumBAA, “PHI” is limited to information received by Business Associate from, or created or received by Business Associate on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103CFR §164.501, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying AgreementEntity (HOPE Family Health).

Protected Health Information. “Protected Health Information” or “PHI” has the same meaning as shall mean the term “protected health information” as defined in 45 C.F.R. § §160.103, provided that, for the purposes of this Addendum, “PHI” and is limited to information the Protected Health Information received from, or received or created on behalf of Covered Entity by Business Associate from, or created or received by Business Associate on behalf of, Covered Entity, when providing pursuant to performance of the Services pursuant to under the Underlying Agreement.

Protected Health Information. “Protected Health Information” or (“PHI” has ”) shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate fromcreated, received, maintained, or created or received transmitted by Business Associate on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has refers to individually identifiable health information, and shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103CFR §164.501, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant . All references to the Underlying AgreementPHI include electronic PHI.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “"protected health information” " in 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate Viva from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof Client.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in Protected Health Information”, as defined by 45 C.F.R. § C. F. R. 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof Humana.

Protected Health Information. “Protected Health Information” (or “PHI” ”) has the same meaning as the this term “protected health information” has in 45 C.F.R. § 160.103CFR §160.103 (as amended by the HITECH Act), provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement. Protected Health Information includes Electronic Protected Health Information.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “"protected health information” " in 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained, or transmitted by Business Associate from, Subcontractor from or created or received by on behalf of Business Associate on behalf of, Covered Entity, when providing the Services pursuant to the Underlying this Agreement.

Protected Health Information. “Protected Health Information” or “(PHI” has ) shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103164.501, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof GSHA.

Protected Health Information. “Protected Health Information” or “" (PHI” has ) shall have the same meaning as the term “"protected health information” " in 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” Section 160.103 and is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof County.

Protected Health Information. “Protected Health health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. CFR § 160.103, provided that, for the purposes of this Addendum, “PHI” is but shall be limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.;

Protected Health Information. “Protected Health Information” " or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate fromcreated, or created maintained, transmitted or received by Business Associate Associate, its agents or subcontractors from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof Department.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. CFR § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained or transmitted by Business Associate from, from or created or received by Business Associate on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has the same meaning as the term “protected health information” set out in its definition at 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained or transmitted by Business Associate from, from or created or received by Business Associate on behalf of, of the Covered Entity, when providing the Services pursuant to the Underlying Agreementand includes “Electronic Protected Health Information (“ePHI”) as defined in 45 CFR §160.103.

Protected Health Information. “Protected Health Information” or (“PHI” has ”) shall have the same meaning as the term “protected health informationProtected Health Information” in set forth at 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has the same meaning as the term “protected health informationProtected Health Information” in at 45 C.F.R. § 160.103106.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained, or transmitted by Business Associate from, from or created or received by Business Associate on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in at 45 C.F.R. § 160.103, provided thatand any amendments thereto, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate MedTrak from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof Client.

Protected Health Information. “Protected Health Information” " or “PHI” has " shall have the same meaning as the term “protected health information” in "Protected Health Information", as defined by 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate fromcreated, or created accessed or received by Business Associate from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof KIC.

Protected Health Information. “Protected Health Information” " or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR §160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or and the abbreviation “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the individually identifiable health information received by Business Associate from, or created or received by Business Associate by, from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof a Participant. Such term shall also include Electronic Protected Health Information.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § CFR 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103CFR §§160.103 and 164.501, provided that, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained, or transmitted by Business Associate from, from or created or received by Business Associate on behalf ofof CHP, Covered Entity, when providing the Services pursuant to the Underlying Agreementand includes Electronic Protected Health Information.

Protected Health Information. “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. CFR § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

CFR § 160.103, provided thatand shall refer to PHI obtained from Covered Entity or created, for the purposes of this Addendumreceived, “PHI” is limited to information received by Business Associate frommaintained, or created or received transmitted by Business Associate on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreementincluding any PHI that is created, received, maintained, or transmitted in an electronic form (“Electronic PHI”).

Protected Health Information. “Protected Health Information” or (“PHI” has ”) shall have the same meaning as the term “protected health informationProtected Health Information” in set forth at 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created created, received maintained or received transmitted by Business Associate on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreement.

C.F.R § 160.103, provided thatand any amendments thereto, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

"Protected Health Information” or “PHI” has " shall have the same meaning as the term “"protected health information” " in 45 C.F.R. § CFR 164.501 and 45 CFR 160.103, provided thatincluding Electronic Protected Health Information, for the purposes of this Addendum, “PHI” is limited to the information received created, received, maintained or transmitted by Business Associate from, from or created or received by Business Associate on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

The term “Protected Health Information” or “PHI” has shall have the same meaning as the term “protected health information” in 45 C.F.R. § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

Protected Health Information. “Protected Health Information” " (or “"PHI” ") has the same meaning as the this term “protected health information” has in 45 C.F.R. § 160.103, provided that, for CFR §160.103 (as amended by the purposes of this Addendum, “HITECH Act) including electronic PHI” . It is limited to information received by Business Associate from, or the PHI created or received by Business Associate from or on behalf of, of Covered Entity, when providing the Services pursuant to the Underlying Agreement.

CFR § 160.103, provided that, for the purposes of this Addendum, “PHI” is limited to the information received by Business Associate from, or created or received by Business Associate from or on behalf of, Covered Entity, when providing the Services pursuant to the Underlying Agreementof Practice.
