The Provider Network Submissions clause establishes the requirements and procedures for healthcare providers to submit information about their network, such as participating physicians, facilities, and services, to a contracting party or regulatory body. Provider Network Submissions. The Contractor shall submit to the Department, in a format and medium designated by the Department, Provider network reports that shall include, without limitation, the following: monthly Provider Affiliation with Sites as set forth in the format given to the Contractor by the Department; monthly updating of all Providers who have either become a Provider in the Contractor's network or who have left the network since the last report; New Site Provider Affiliations as new Sites are added; Site terminations immediately as they occur; and Enrollee Site Transfers as they occur. New Site/PCP information shall be reported in a format and medium as required by the Department. During the term of this Contract, this report may be converted to electronic data transmission. The Department will give the Contractor no less than one hundred twenty (120) days notice prior to conversion of this report to electronic data transmission. Such notice shall advise the Contractor of the submission requirements.
