Rating of Employee Upon Resignation. In the event an employee gives notice of an intention to resign, the employee may request an evaluation on the form for the regular Final Evaluation Report. This evaluation shall be completed prior to the effective date of the resignation, provided the employee has made the request at a time which reasonably permits the evaluator to do so.
