Common use of Recording of Designated Disclosures of PHI Clause in Contracts

Recording of Designated Disclosures of PHI. BA agrees to maintain and make available information required to provide an accounting of disclosures to FHKC as necessary to satisfy FHKC’s obligations under 45 CFR § 164.528. BA agrees to provide to FHKC, within fifteen (15) days and in a secure manner, information collected in accordance with this provision, to permit FHKC to respond to a request by an Individual for an accounting of disclosures of PHI in accordance with 45 CFR § 164.528 and applicable state law.
