Recordkeeping and Reporting Requirements. 7.6.1. The Contractor must maintain updated Grievance and Appeals system information and abide by all Grievance and administrative hearing decisions. 7.6.2. The Contractor must be able to submit upon request Individual-level information in a spreadsheet format to Great Rivers that will contain at least the following information regarding each grievance received by the Contractor. 7.6.2.1. Clients full name, date of birth, and P1 or CIS identifier. 7.6.2.2. Date and type of grievance. 7.6.2.3. Outcome of review of grievance process. 7.6.2.4. Plan for improvement, if patterns of grievance are identified. 7.6.3. Reports that do not meet the grievance system reporting requirements shall be returned to the Contractor for correction. Corrected reports must be resubmitted to Great Rivers within thirty (30) calendar days. 7.6.4. The Contractor must submit a report in a format provided by Great Rivers that includes: 7.6.4.1. The number and nature of Grievances. 7.6.4.2. The timeframes within which they were disposed of or resolved. 7.6.4.3. The nature of the decisions. 7.6.4.4. A summary and analysis of the implications of the data, including what measures will be taken to address undesirable patterns. 7.6.5. Using the form provided by Great Rivers, the Monthly Grievance report, is due to Great Rivers within twenty (20) days following the end of the month. 7.6.6. Using the form provided by Great Rivers, the Quarterly Grievance report is due to Great Rivers within twenty (20) days following the end of each calendar quarter.
