Recordkeeping Requirements of Grievances and Appeals. For purposes of quality review, the Contractor shall accurately maintain records for grievances and appeals that contain, at minimum, the following information: A general description of the reason for the appeal or grievance; The date the appeal or grievance was received; The date the appeal or grievance was reviewed; The resolution of the appeal or grievance; The date of the resolution of the appeal or grievance; and The name and MID number of the member for whom the appeal or grievance was filed. The Contractor shall provide such record(s) of grievances and appeals upon request by OMPP.
