Records Upon Termination. United, Subcontractor and Provider recognize that in the event of termination of the State Contract for any of the reasons described therein, Subcontractor and Provider shall immediately make available to United, in a usable form, any and all records, whether medical or financial, related to Subcontractor’s and Provider’s activities undertaken pursuant to the Agreement and the State Contract so that United can immediately make available the same to LDH or its designated representative. The provision of such records shall be at no expense to the Department.

Records Upon Termination. All records and information provided by the Board or through its third party contractors to the Auditor are the sole property of the Board and shall be returned, to the Board within thirty (30) days of the termination date of this Contract. Notwithstanding anything herein, the Board acknowledge and agrees that the Auditor may retain an archival copy of all records and information provided to the Auditor by the Board or its agents to support the Auditor’s work under this Contract and to comply with the Auditor’s business continuity and document retention policies, subject to the Auditor’s continued compliance with its confidentiality obligations under this Contract.

Records Upon Termination. Upon the effective date of termination of this Agreement or revocation of all Delegated Activities pursuant to Section 3.3, Vendor shall provide United with a list of all Participating Practitioners and Participating Components that Vendor has Credentialed on United’s behalf. Also, upon request by United, and if agreed to by Vendor, Vendor shall provide United with copies of Vendor’s Credentialing and Recredentialing files that pertain to this Addendum. Such files shall be provided to United no more than thirty (30) days after the effective date of termination of this Agreement or revocation of all Delegated Activities pursuant to Section 3.3 of this Addendum.

Records Upon Termination. Upon termination of this Agreement, FACILITY agrees that all documents, records, notebooks and similar repositories of or containing Secret Information, including copies thereof, then in the possession of the FACILITY, or any employee, agent, independent contractor or other persons or organizations over which the FACILITY has control, whether prepared by the FACILITY or others, shall be returned to PROHEALTH, except for patient health records of individual patients of the FACILITY.
