Rehabilitative and habilitative services. Benefits are provided for the following forms of rehabilitative and habilitative therapy: 1. Physical Medicine Benefits for physical medicine (which also includes pulmonary therapy, orthoptic therapy, and urinary incontinency therapy) include evaluation and treatment by physical means or modalities, such as: mechanical stimulation, heat, cold, light, air, water, electricity, sound, massage, mobilization, and the use of therapeutic exercises or activities performed to relieve pain and restore a level of function following disease, illness or injury.
