Common use of Reimbursement for Approved Facility Clause in Contracts

Reimbursement for Approved Facility. Subject to the limitation in Subd. 3, below, reimbursement for the cost of a single membership at an Approved Facility(s) shall be made once per year to any employee who, between January 1 and January 31 of the following year, submits proof of the cost of the membership during the preceding calendar year. Reimbursement payments shall be made to employees on or before the last day of February, subject to normal withholding for taxes. Proof of cost may be made by invoices, account statements or verification from the Approved Facility. An employee who separates from service during the calendar year may seek reimbursement, subject to the limitation, for the cost of the membership during the year up to and including the month in which the employee separates from employment.
