REPORTS OF. SUDRF CHANGES The governing body or the administrator of the facility shall submit in writing to DHA, any significant proposed changes within the SUDRF no later than 30 days prior to the actual date of change. A report shall be made concerning the following items: (a) Any change in administrator or primary professional staff.
