BUSINESS ASSOCIATE agrees to report to COVERED ENTITY, and any other required entity under HITECH if there is a BREACH of protected health information whether a mistake or intentional, any knowledge, discovery, or identification of use or disclosure of INFORMATION not permitted by the PRIVACY REGULATIONS, HITECH, and this CONTRACT.
