REPORTS, RECORDS AND AUDITS. A. The Plan shall submit a monthly report to Preferred Life listing the names and amounts for those Members who have received Eligible Hospital Services during the Contract Year which exceed seventy-five (75) percent of the per Member Deductible as set forth in Article II.
