Request for New Services. (a) Any Xxxxx Xxxxxx Entity may submit, from time to time, a written request having the form of Schedule 2.02(a) and signed by a duly authorized representative of Xxxxx Xxxxxx to the GE Digital Services Manager for the initiation of a New Service to be provided by GE Digital (each, a “Request for New Services”). Each Request for New Service shall be deemed to incorporate by reference the terms and conditions of this Agreement and be numbered and dated. Requests for New Services shall contain the following information to facilitate the proper accounting of such Service through the intercompany billing system:
