Requests for Sick Leave. ‌ When the employee’s need to use sick leave is foreseeable (for example, prescheduled appointments), the employee must provide reasonable advance notice to their immediate supervisor of the need to use sick leave and its probable duration, if known. In all other situations, an employee who must be absent from work shall personally (if able) notify their immediate supervisor as soon as reasonably possible, and no later than the employee’s scheduled start time.
Requests for Sick Leave. The employee shall furnish the Employer a request to justify the use of sick leave. The payment of sick leave is contingent upon the approval of such request by the Employer. The nature of the cause of sick leave request must be explained by the employee. If an employee is expected to be absent for more than three (3) consecutive business days (on either paid sick leave or unpaid leave of absence due to illness) the employee shall submit a written statement from their medical practitioner for the leave request to be considered. The required statement shall give the reason for the anticipated absence and the expected date of return to work of the employee.

Request for sick leave in all other cases shall be made in advance whenever it is possible, no later than within the first (1st) hour of the start of the employee's workday. B. Employees covered under this Agreement, who provide up-to-date medical documentation (biweekly) stating that the employee cannot perform their regular assignments will be provided light duty assignments when such assignments are available within the Department.

Callouts for the next workday should be made Sunday through Thursday between 7:00 PM and 8:00 PM. The evening hour is only for requested sick leave the following workday, not regular business. C.
