Common use of Required by DHS Not required by DHS Clause in Contracts

Required by DHS Not required by DHS. Professional Liability Insurance covering any damages caused by an error, omission or any negligent acts related to the services to be provided under this Agreement. Provider shall provide proof of insurance of not less than the following amounts as determined by the DHS: Per occurrence limit for any single claimant: From commencement of the Agreement term through June 30, 2015: $1,000,000. From July 1, 2015 and every year thereafter, the adjusted limitation as determined by the State Court Administrator pursuant to ORS 30.271(4). Annual aggregate limit for multiple occurrences and multiple claimants: From commencement of the Agreement term through June 30, 2015: $3,000,000. From July 1, 2015 and every year thereafter, the adjusted limitation as determined by the State Court Administrator pursuant to ORS 30.271(4).
