Required Program Participant Survey. The AGENCY shall develop a survey(s), in a format approved by the COUNTY, to elicit feedback from program participants, parents, and/or stakeholders to assist in the enhancement of the AGENCY’s delivery of the Services. Surveys shall be administered at least two (2) times a year to program participants. A copy of the compiled survey results and comments shall be submitted during the 2nd and 4th Quarter.
