Requirements for Medical Verification. to meet the requirements of Section (e) above, a medical certificate must: (1) be signed by a person licensed to practice medicine; (2) describe briefly the nature of the illness or injury which resulted in the employee's absence; (3) state the date or dates on which the person signing the certificate treated the employee for the illness or injury which resulted in the employee's absence; (4) state any duties and responsibilities that the employee is incapable of performing with the medical condition treated; and (5) state that the employee is fit to resume the duties of his/her position.
