Restrictions on Use and/or Disclosure of Protected Health Information. Contractor shall comply with all granted restrictions on the use and/or disclosure of Protected Health Information, pursuant to 45 C.F.R. § 164.522(a), upon notice from Physician Practice. Contractor shall forward to Physician Practice any requests for restriction on the use and/or disclosure of Protected Health Information within five (5) business days of receipt.
