Retention of Responsibility. The Nursing Home agrees that Hospice retains the responsibility for planning, coordinating, prescribing, and documenting Hospice services and care on behalf of the patient and the family of the patient and the Nursing Home agrees that Hospice retains responsibility for appropriate hospice care training of the personnel who provide the care under this Agreement.

The Nursing Home agrees that Hospice retains responsibility for ensuring that the training of personnel who will be providing the patient’s care in the inpatient facility has been provided and that a description of the training and the names of those giving the training are documented. This is for appropriate hospice care training of the personnel who provide the care under this Agreement.
