Routine Care. Within twenty-eight (28) days. Non-symptomatic office visits shall include but shall not be limited to: well/preventive care appointments such as annual gynecological examinations or pediatric and adult immunization visits.

Routine Care. For routine dental care detailed below, the Plan reimburses eligible employees 90% of the Ontario Dental Association Fee Guide allowance, on a one year lag basis, for the eligible expense incurred by the employee and each eligible dependent. - oral exams, including the cleaning of teeth, but not more than once every 6 months; - root planing or equilibration (limited to 8 units per year for all procedures combined); - periodontal scaling; - topical applications of sodium or stannous fluoride (where such application is necessary for the maintenance of sound dental health); - dental x-rays; - fillings; - extractions; - oral surgery, including excision of impacted wisdom teeth; - reasonable and customary charges for anesthesia and its administration; - repair, relining or rebasing of dentures; - space maintainers, including stainless steel crowns for baby teeth that have several cavities which would otherwise require filling or which are non-restorable using normal restorative dental material; -caries susceptibility test (Cariescreen); - in-office topical fluoride (Fluorzion Foam); - topical antimicrobial (Chlorzion); - dispensing of fluoride (Fluorzion Homecare); - consultants (at the start of treatment only); - microbiological test ( Bana test); - pit and fissure sealants; - periodontic treatment for disease of the bone and gums of the mouth, including tissue grafts and occlusal guards, but not athletic guards; and - endodontic treatment, including root canal therapy.
Routine Care. The Adopter agrees to take the Animal to a veterinarian for regular examinations, and vaccinations as your vet determines and to procure immediate veterinary care, at your own expense should the Animal become ill or injured. Routine Care. As described above, Inmates may be identified as having mental health and/or substance use disorders that require routine behavioral health care. Routine care shall include a continuum of evidence-based cognitive behavioral psychosocial interventions (individual and group) as well as psychotropic medication. Provider’s behavioral health treatment philosophy for Inmates should be structured in such a way as to support an Inmate clinically moving up and down the continuum based on clinical need and to support an Inmate’s recovery. The service continuum for mental health and substance Services shall be delivered as follows: For Inmates with mental health diagnoses without co-occurring substance use disorders that require treatment intervention, Provider shall provide a) outpatient, b) intensive outpatient and c) residential mental health Services at all Level V facilities. Outpatient treatment will generally involve 1 to 4 contacts per month and intensive outpatient treatment will generally involve multiple contacts per week. Inmates receiving outpatient and intensive outpatient Services will typically reside with the general population. Provider will use evidence based interventions and evidence- based curricula regarding treatment strategies. In accordance with the CLASI v. Coupe, Provider will also operate a mental health residential program within a designated housing area for Inmates with SMI whose symptoms and/or functioning prohibit them from being able to live within the general population at each facility. The residential program will operate 7 days per week with a minimum of 10 hours of structured therapy provided Monday through Friday (may be a combination of individual and group) and additional therapeutic activities provided daily and on weekends for those in need of mental health residential care. Provider will submit a schedule of for the anticipated programming for the residential treatment program. At the Xxxxx X. Xxxxxx Correctional Center (“JTVCC”), Provider will operate the established Residential Treatment Center (Building 21) consistent with the 2016 CLASI v. Coupe settlement. Provider shall provide outpatient and intensive outpatient Services (similar to those in Level V facilities) for the Level IV facilities. Routine care is that which can be administered at the facility by VMDC staff. Routine and ordinary care EXCLUDES pharmacy and prescription services, lab tests, x-rays, specialist consultation, treatment of severe chronic or life-threatening maladies, surgical procedures, hospital stays, and other similar care. Detainees transported to VMDC shall arrive with medical clearance and a brief medical history.
