Second Qualifying Event Extension Sample Clauses

Second Qualifying Event Extension. If a second qualifying event occurs while receiving eighteen (18) months of COBRA continuation coverage, an enrolled spouse and Dependent children can qualify for eighteen (18) additional months of COBRA continuation coverage, for a maximum of thirty-six (36) months, if notice of the second qualifying event is properly given to the Plan. This extension may be available to the spouse and any Dependent children receiving COBRA continuation coverage if the Subscriber or former Subscriber:  dies;  becomes entitled to Medicare benefits (under Part A, Part B, or both);  gets divorced or legally separated; or  if the Dependent child no longer qualifies as a Dependent child as provided in Section 1. of this Certificate.
