Sentinel Event. Any unexpected event or occurrence involving death or serious physical injury or psychological injury, or the risk thereof. Serious injury includes loss of limb or function. The phrase “risk thereof” includes any process variation for which a recurrence would carry a significant chance of a serious adverse outcome.

Sentinel Event. A Sentinel Event is an unexpected occurrence involving death or serious physical or psychological injury which may include loss of limb or function. A Sentinel Event may include homicide, attempted homicide, suicide, attempted suicide, or sexual assault and is more fully described in the Lucet Provider and Facility Manual.

  • Contract Termination; Debarment A breach of the contract clauses in paragraph 1 through 10 of this section may be grounds for termination of the contract, and for debarment as a contractor and a subcontractor as provided in 29 CFR 5.12.

  • Program Termination In the event the Utility’s Electric Security Plan (“ESP”) or Market-Rate Offer is terminated prior to the end of this agreement, this agreement shall automatically terminate.
