Service Area, Generally. (A) The Service Area is the specific geographic area within which the Medicaid Eligible Individual must reside to enroll in the Contractor’s Health Plan. The Service Area for this Contract is all counties within the State of Utah. (B) The Contractor shall provide adequate assurances and supporting documentation that the Contractor has the capacity to service the expected enrollment in the Service Area.

The Service Area for this Contract is Salt Lake County. (B) The Contractor shall provide adequate assurances and supporting documentation that the Contractor has the capacity to serve the expected enrollment in the Service Area.

The Service Area for this Contract is the entire state of Utah. CHlP Eligible lndividuals in all counties of the State of Utah are mandatorily enrolled in a CHlP plan.
