Service Area, Generally. (A) The Service Area is the specific geographic area within which the Enrollee must reside to enroll in the Contractor’s Health Plan. The Service Area for this Contract is the entire state of Utah.

The Service Area for this Contract is in the following counties: Salt Lake, Utah, Weber, Davis, Tooele, Summit and Wasatch.
