Service Provision. Subject to, and in accordance with, the terms and conditions in this Agreement, the Operator will provide the Services (either directly or by making its facilities and services available to other Physicians) to: (a) those Clients of Physicians who arrange for use of the Facility for the purposes of providing surgical services to such Clients, or (b) in cases only where referral by a Physician is not required, those Clients who present themselves directly to the Operator and who request the Services, provided that in each instance, the provision of the Services requested is both clinically and ethically appropriate, constitutes the provision of a service which is Insured to a person eligible to receive those Services and is provided during the Term.

Service Provision. 3.1 Voip-Unlmited will notify the Reseller of the ECCs as soon as reasonably practicable after the date of the Service Order Form. 3.2 Any works carried out outside of business hours will incur an additional charge. 3.3 On receipt of the notice referred to at clause 3.1, the Reseller may within thirty days notify Voip- Unlimited that the ECCs are accepted and it wishes to proceed with the Order insofar as it relates to the provision of Ethernet Services. 3.4 If the Reseller does not serve any notice under clause 3.3 within thirty days, unless the ECCs are waived as set out at clause 3.4hereof, the Order insofar as it relates to the provision of Ethernet Services, shall be cancelled by Voip-Unlimited. Such cancellation will not affect any agreement between the parties to provide any other Services save where provision of those Services is dependent on the provision of Ethernet Services in which case they shall also be cancelled. 3.5 Voip-Unlimited may elect at any time, which includes for the avoidance of doubt before or after the Reseller has served a notice pursuant to clause 3.3 hereof and / or before or after the expiry of the thirty-day period referred to above, in its absolute discretion, not to charge the ECCs to the Reseller in which case the Order shall proceed and shall not be cancelled. 3.6 Pricing and lead times are subject to site survey and may vary if there is insufficient bandwidth capacity at the premises or if additional work is necessary including but not limited to provision of new fibre, duct work, or building entry points. Any ECCs will be advised after completion of the site survey. 3.7 If the site is unable to be accessed for any of the following reasons, then the Reseller will pay charges calculated in accordance with Voip-Unlimited’s Price List and Tariff: 3.7.1 The engineer is unable to access the site; or 3.7.2 The site preparation has not been carried out in accordance with Health and Safety recommendations; or 3.7.3 The engineer is unable to contact the specified technical contact; or 3.7.4 the Reseller fails to complete site preparation in accordance with all current legislation, including but not limited to health and safety and including the production of an asbestos register if required. 3.8 In order for Voip-Unlimited to provide the Reseller with a service, the Reseller must ensure that 3.8.1 There are two 13-amp power sockets that are available within 1 metre of the termination location 3.8.2 There is enough space for the...
Service Provision. 5.1 Customers have a choice on how they would like their Voip exchange product configured, however the customer is responsible for completing the technical aspect form; the Voip exchange product will be configured according to the submitted technical aspect form and any deviation from this form, required to be completed by Voip Unlimited, will be subject to a reconfiguration fee in the sum set out on Voip Unlimited’s Price Lists and Tariffs. 5.2 The Handsets will not be ordered for delivery until a signed valid service order form and a technical aspect form have been received and approved by Voip Unlimited.

Provide the family information and support in referring to and accessing Youth Empowerment Services (YES) Waiver and offer the child/youth and family Wraparound services in a) LOC-4 Intensive Family Services; or

Service Provision. A2.1 You must provide the Services for which you receive payment under this Agreement: a. in a prompt, efficient, professional and ethical manner; b. in accordance with all relevant law; and c. in accordance with the service specifications set out in Section D and Section E, from the Commencement Date without material interruption until this Agreement is terminated in accordance with the terms of this Agreement. A2.2 You must use your best endeavours to provide the Services in a manner that is consistent with any relevant health strategy, disability strategy and any strategy for the development and use of nationally consistent standards, quality assurance programmes and performance monitoring determined by the Minister of Health under section 8 or section 9 of the NZPHD Act, except to the extent that any such strategy is inconsistent with this Agreement.

Lead times for simultaneous provides are subject to BT survey and can exceed 10 working days and therefore Reseller Required by Date cannot be guaranteed. 5.2 Openreach may reuse an existing socket and line routing to fulfil your order; if this occurs your appointment will be cancelled and no engineer will attend site, therefore by selecting “Forced Provision” you are accepting Time related charges for an Engineer Visit calculated in accordance with Voip- Unlimited’s Price List and Tariff.

Service Provision. 1. Grantee shall ensure that all enrolled individuals who are interested have access to Individual Placement and Support Services (IPS) regardless of readiness factors, substance abuse, symptoms, history of violent behavior, cognition impairments, treatment non-adherence, or personal presentation. 2. IPS or SEES Specialist shall assist enrolled individuals in pursuing permanent competitive jobs and academic opportunities in mainstream, integrated educational settings. Acceptable jobs include seasonal jobs and temporary jobs that are part of the labor market. 3. Grantee shall provide a 24/7 phone access for enrolled individuals and their families to use when a crisis occurs. Grantee shall provide in-person crisis support through an established Local Mental Health Authority/Local Behavioral Health Authority (LMHA/LBHA) to help connect individuals to necessary resources, including medical backup. 4. As required in the applicable Utilization Management Guidelines, Grantee shall regularly assess all individuals during intake for suicide risk and thereafter proactively monitor suicide risk throughout treatment and whenever safety concerns arise. Safety Plans must be developed and implemented by CSC-FEP program staff for individuals identified by the staff to be a safety risk within 24 hours. Grantee shall use System Agency-approved risk assessment tools and file Safety Plans must be made available to System Agency upon 5. As defined in the applicable Utilization Management Guidelines, Grantee shall develop a Recovery Plan during the admission process and every 90 calendar days thereafter. CSC-FEP team shall periodically discuss with each enrolled individual his or her preferences for family involvement. 7. Grantee shall ensure that the length of stay in the CSC-FEP program for any individual does not exceed 36 months. 8. CSC-FEP program shall document efforts to support clients in meeting basic needs by directing clients to community support resources and helping individuals complete the required steps to access available resources. 9. Services will be offered as ‘Lots9’ for the purpose of establishing a commercial Framework. It is recognised that not all IDPs will (in isolation) be able to provide services across the full range of service delivery channels (E-Channel, telephone and face-to-face).
