Common use of Services from Unlicensed or Ineligible Providers Clause in Contracts

Services from Unlicensed or Ineligible Providers. The Plan does not provide Benefits for services received from Providers that are not licensed by law to provide Covered Services. Examples of such Providers may include, but are not limited to, masseurs or masseuses (massage therapists) and physical therapist technicians. The Plan does not provide Benefits for services provided by any Provider not listed as an eligible Provider in this Agreement.
