Common use of Services Request Form Clause in Contracts

Services Request Form. 2.3.1.1. The Judicial Council’s Project Manager will provide Contractor with an unsigned Services Request Form (Attachment B), describing the Work the Judicial Council wants performed. The Judicial Council’s Project Manager will complete the Services Request Form and send to Contractor electronically. The Judicial Council’s Project Manager will inform the Contractor of the appropriate Pricing Methodology (i.e. Lump Sum Based Pricing or Time and Materials Pricing) for the Project.
