Statement of Furnishing. A sworn statement identifying: (i) the names of all parties furnishing and the goods, labor, or services so furnished to the Project with a value in excess of twenty-five thousand dollars ($25,000) during the time period of the applicable Application for Payment; (ii) payments made to each party furnishing goods, labor, or services; and (iii) amounts due and remaining amounts that are likely to become due to each party furnishing goods, labor, or services.
